Friends and Family

Our surgery relies on the opinion of our patients so that we can improve our services. We have an Elephant Kiosk in the practice waiting area which patients can use to write their views. we also have a paper form which is on our front desk which you can fill out and place in the Friends and Family suggestion box. All questionnaires are anonymous and can be filled at any time.

Visiting the UK? You may be chargeable for NHS Treatment.

If you are not ordinarily resident in the UK then you are classed as an overseas visitor and will have to pay for most treatment outside the GP practice, unless exempt.

HMR Circle.

HMR Cirlce is a membershp group for adults in the Heywood, Middleton adn Rochdale area. Circle is about learning new things, and enjoying your hobbies and interests with others in the community. One number to call to help stay sorted, and a social network to help stay connected, that's the idea.

MJOG text messaging service


The surgery now has a text messaging service so that we can remind you of your appointment. We will send text messages out 7 days before and 24hrs before your appointment. Please be considerate to other patients and cancel if you are unable to attend so the appointment can be offered to another patient. You can cancel your appointment via the text messaging service.

R U Clear


RUclear is run by the NHS, the tests are of a high quality and are free. It offers free and confidential postal tests for people aged 16 to 24 years, chlamydia and gonorrhoea.
If you would like further information on how to receive a testing kit, please click here where you can request a kit to be sent to your home.

Children Absence from School

If your child is absent from school due minor ailment, please complete the form below which is a confirmation that your child has required time off due to an illness as managed by yourself but has not been reviewed by a registered GP. This form can then be submitted to your child's school.
